GlossaryMethodology

FMEA (Failure Mode and Effects Analysis)

A systematic method for identifying potential failure modes, their causes, and effects. Uses Severity, Occurrence, and Detection ratings (1-10) to calculate a Risk Priority Number (RPN = S x O x D).

Key Benchmarks

RPN threshold: >100 requires action
Typical failure modes: 20-50 per process
Risk reduction target: 50-80%
